So you've gotten to the point where you
and/or the school believes an evaluation is in order.
In a letter to the school's principal,
indicate your desire for a CORE Evaluation. The letter does not have to
be fancy; it just has to have the appropriate wording.
SAMPLE LETTERS
Dear Mr. _______________,
We have met several times thus far to
discuss my daughter, M's progress. Because it seems that we all have
some concerns, I am writing to request a CORE Evaluation of my daughter
M. I understand that the district has five days to send me consent to
evaluate my daughter M. I look forward to your timely response.
Dear Ms. _______________,
My son, J is a third grader at your
school. He is really starting to struggle this year with attentional
issues, and I find him dropping further and further behind his peers. I
am writing to request a CORE Evaluation. I understand that the district
has five days to send me consent to evaluate my son J. I look forward
to your timely response.
Dear Mrs. ______________,
My daughter S is in the eighth
grade. Although she is admittedly very bright, we find that she is
struggling to keep up with her peers. We are writing to request a CORE
Evaluation. That evaluation should include cognitive as well as psychological
testing. We understand that the district has five days to send us
consent to evaluate our daughter S. We look forward to your timely
response
